How to Request Your Health Care Records

Current and former clients may request copies of their counseling and mental health records. You may submit your request using any of the following methods:

Email (preferred): Admin@centextherapies.com
Mail: 3100 S W S Young Dr #10333, Killeen, Texas 76542

Please include the following in your request:

  • Client full legal name and date of birth
